To bill and receive payment for the services you receive, including preparing insurance claims and providing superbills upon request.

To improve quality of care, train staff, and manage our practice.

When Required by Law

We may disclose information when required to comply with federal, state, or local laws, including public health reporting and law enforcement.

3. Other Uses and Disclosures Require Authorization

We will ask for your written authorization before sharing your PHI for purposes not covered in this Notice, including:

  • Marketing communications

  • Sharing PHI with third parties for purposes unrelated to your care

  • Most disclosures of psychotherapy notes

You may revoke your authorization at any time in writing.

4. Your Rights Regarding Your Information

You have the right to:

  • Request Amendments: Ask us to correct or update information you believe is inaccurate.

  • Request Confidential Communications: Choose how we contact you (e.g., personal email vs. secure platform).

  • Restrict Disclosures: Request limits on how your PHI is shared — though we may be required to comply with certain laws.

  • Receive a List of Disclosures: Ask for a record of when your PHI has been shared.

  • Obtain a Paper Copy: Request a paper copy of this Notice at any time.

5. Telehealth Privacy

We offer secure, HIPAA-compliant telehealth consultations. While we use encrypted platforms to protect your information, you are responsible for ensuring your environment is private and secure during virtual visits.

6. Confidentiality & Security Measures

  • HIPAA-Compliant Platforms: We use IntakeQ for scheduling, intake forms, telehealth visits, and secure messaging.

  • Secure Email Practices: We use Gmail for non-PHI communication only.

  • Website Contact Form: Do not include PHI in Squarespace’s contact form — use our secure IntakeQ booking system.

7. Insurance & Billing

  • We are an out-of-network provider and do not bill insurance directly.

  • Superbills are available upon request for potential reimbursement.

  • We are not enrolled as a Medicare provider; call us if you are a Medicare patient.

8. Third-Party Services

We may use third-party vendors to support our practice, including:

  • IntakeQ (HIPAA-compliant booking & telehealth)

  • Squarespace (website hosting)

  • GoDaddy (domain management)

  • Gmail (non-PHI communications)

All third-party vendors with access to PHI are required to sign Business Associate Agreements (BAAs) or meet equivalent HIPAA standards.

If you believe your privacy rights have been violated, you can:

  • We will not retaliate against you for filing a complaint.
